Lucy Hill Services is a medicaid provider registered with the Virginia Department of Medical Assistance. The Elderly or Disabled Consumer Direction (EDCD) Waiver offers an alternative to placing your loved one in a nursing home or assisted living facility. This assistance includes personal care (whether provided by you or an agency) and respite care (assistance provided to give you a break from providing personal care to your family member). You're entitled to apply for this service if you are eligible for Medicaid or are enrolled in the new Commonwealth Coordinate Care Program. The PAS (pre-admission screening) Team determines whether the individual is eligible for the EDCD Waiver services. The screening process is a coordinated effort between the Social Worker at the hospital or treatment facility, Adult Protective Services Unit at your local Department of Social Services, the registered Nurse from the local Health Department and the client. The Department of Medical Assistance Services (DMAS) or its service authorization contractor must authorize all waiver services based on the individual's needs. The consumer must meet nursing home criteria or be in imminent danger of placement if in-home services aren't started within 30 days.
As your Service Facilitator/Provider, I will submit the necessary documents to KePRO and PPL and coordinate the services needed immediately.
